As a means to better assist and treat our patients, Dermatology and Skin Cancer Center has upgraded their electronic medical record system. The EMA Dermatology portal is a secure patient portal which is compliant with the new health care regulations. At the EMA Dermatology portal, our patients will have the ability to view office notes, laboratory results and pathology reports. EMA Dermatology also allows patients to request medication refills and send secure messages to our medical staff.
Starting September 1, 2014, you will be asked to enroll in our new patient portal during your office visit. We will create a unique and secure username and password once you provide us with an email address. Log on to obtain your personal patient information by clicking on the EMA Dermatology link to the left of this Web page.
If you were enrolled in our patient portal system before September 1, 2014, you may call the office to obtain a new secure username and password for our new system.
Unfortunately, at this time our new portal does not allow patients to make appointments or view and pay statements. You may request appointments through the new portal by sending a secure message to our staff. Please leave a preferred phone number so we can call you back to schedule the appointment. For any billing questions, you may call our office and select Option 3 to speak with our billing department.

In order to expedite your first visit with our office, please print the New Patient paperwork below. Bring completed forms along with your current insurance card, photo ID and arrive 20 minutes prior to your visit. If you have been referred by another physician please bring pertinent medical records, biopsy or lab results and referral if required by your insurance carrier.
